Green Card Medical Exam
To keep your time in our office to a minimum, we are asking all our clients to fill out these intake forms before coming in to see us. Please answer the following questions to the best of your ability. If you do not understand a question, leave it blank and our medical interviewer will go over it with you later.
Once you have completed these forms, please send pictures of any immunization records you have to [email protected] and we will go over them during your interview.
